As a Condition of Employment: 1. I hereby authorize Deviney Construction to conduct an investigation concerning all statements contained in my application for employment, to take my fingerprints, to interview all references and employers and to conduct any other investigation that it deems appropriate. I request only duly constituted law enforcement agency or judicial office to furnish the company with all information it may have pertaining to me and I hereby release Deviney Construction and law enforcement agency, judicial officer or other individual from any liability arising from the disclosure of any information pertaining to me which is obtained during said investigation. 2. In making this application for employment, I also understand that an investigative consumer report may be made whereby information is obtained through personal interviews by neighbors, friends or others with whom I am acquainted. This inquiry includes information as to my character, general reputation, personal characteristics and mode of living. I understand that I have the right to make a written request within a reasonable period of time to receive additional detailed information about the nature and scope of this investigative consumer report. 3. I understand that any false statement or omissions of information in this application will be sufficient for termination of employment. 4. I understand that Deviney Construction reserves the right to require its employees to submit to blood tests or urinalysis for alcohol or drug screens (except where prohibited by law). I hereby give permission for a complete physical examination, if requested by Deviney Construction, including x-rays, blood tests, urinalysis, and consent to the release of any and all information as may be deemed necessary by Deviney Construction. 5. By submitting the application, I agree that, absent further written agreement to the contrary, if I am accepted for employment by Deviney Construction, I will thereafter, in consideration of my employment communicate, convey, and assign Deviney Construction all of my right, title and interest in and to any and all novel ideas, concepts and inventions, patentable or not and any patent applications thereon, which relate to the business of operations of Deviney Construction and which are made or conceived by me either solely or jointly with others during my employment term, and I will neither disclose to any other person nor use for my own personal benefit any information relating to the business of Deviney Construction, not publicly known, which is acquired by me during the term of and arising out of my employment. By submitting this application, I further understand and agree that my employment is for no definite period of time and may, regardless of the date of payment of wages and salary, be terminated at any time without any previous notice.
